          Sub:- Request for retention / modification on medical grounds in favour of Sh. Vivek Kumar Teacher transferred vide order No. XXXXXXXXX dt 18/8/2016 .

          In all reverence and humble submission I wish to inform your goodself that I have been transferred from Boys Hr Sec. School Satwari to Hr Sec. School Rehal vide your order No. XXXOJof2016 dt. 18/8/2016 . Sir I would like to apprise your honour that I am suffering from LBA PIVD (Disc Problem) and the standing medical board has also certified my suffering and recommended some instruction   vide there order NO. XXXXXXXXXXXXX dt. 22/08/2015 . Sir I have also from time to time submitted the same in your office as well as also to the authority appointed by your good self for seeing the request for effecting the transfer in the transfer mela . I am not against the transfer but at the same time I would like to request your good self that I may be transfer to the school as given by me in the choice Performa or any other school in Satwari. Sir as there is no convenience  from Satwari to Rehal  and neither I drive any vehicle  So It is requested to your good self that either I should be retained or I should be transfer to any of the under mentioned school .

              GHSS Satwari ,GHS Bhatayari, MS Gangyal, HS Deoli MS Dhinday Khurd, MS Kanhal.Copy of  the medical Certificate is enclosed for your reference and record .Hoping an early action in this matter.
